1. When should I arrive?
If this is your first visit to Eterna Vein & Medical Aesthetics, please check-in at least 30 minutes prior to your appointment to fill out appropriate paperwork; otherwise, you can fill out the patient intake forms located on our website and bring them to your scheduled appointment.
For those established patients, check-in time should be 10 minutes prior to your appointment.
2. What items should I bring with me?
- Insurance cards
- Photo identification card
- Vein ablation patients need to be accompanied by a driver; car services or taxis are not allowed
3. What forms of payment do you accept?
Payment is due at time of service. Major credit cards, cash and personal checks are accepted. CareCredit also accepted.
4. What if I am late for an appointment?
It’s possible that we may have to shorten or re-schedule your appointment. If your treatment is shortened, it will end on time so that the next patient will not be delayed. We appreciate your understanding.
5. What if I need to cancel an appointment?
These things happen! If you find yourself unable to keep an appointment, we require a 24-hour notice. Patients who do not honor their appointments or cancel in less than 24 hours will be subject to a $50.00 cancelation fee.
6. Are children permitted in the clinic?
To protect the safety of the child due to the procedures that we provide we cannot allow children in the treatment rooms. For the comfort of all of our patients and the enjoyment of your experience, children cannot be left unattended at any time.
7. Do you allow cell phones?
Please silence your cell phones when you enter the office in order to maintain our serene atmosphere. To expedite your treatment, once in the exam room, we request that you turn off your cell phone.
8. What does a cosmetic consultation cost?
If you are visiting our clinic for a cosmetic consultation, there is no fee for the consultation. We believe that building relationships is important to your care and satisfaction. For this reason, we grant complimentary consultations to our cosmetic patients to meet our staff and discover the treatment options that are available. We book 30 minutes for our complimentary cosmetic consultations.
9. What if I am traveling to your office from out of town?
There are 2 hotels that are conveniently located for our guests who are traveling from out of town.
